Sports Medicine Institute accepts all PPO/POS private insurance plans. We do not accept any HMO, Medicaid, Medi-cal, or liens.

We are a preferred provider for the following insurance companies. *Please verify with specific SMI location as to whether they are a preferred provider or not.

If you do not see your insurance company listed please call and talk to our front office to determine if we will accept out of network benefits.
